Patient Forms
In order to schedule a new patient appointment, please fill out our
required new patients forms below and call us at (757)838-9140 to
schedule your appointment. Please note we cannot schedule an appointment
until you are registered in our scheduling system.
Any missing required fields will prohibit the submission of the forms
and prompt you (in red) to complete the missing fields prior to a
successful form submission. Please call our office at (757)838-9140 once
your forms have been completed to schedule a new patient appointment.

2) Select the "Submit" button after completing each form
3) A message will appear if your forms were successfully sent

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all
of the needed information to provide the best possible care for you. It also
will help relieve any unnecessary anxiety you may be feeling. Please be
prepared to bring your government-issued form of ID and your dental
insurance card.
If you’d like to do this paperless, you can also e-mail a picture of the
